New Grants Help Hospitals Promote Long-Term Care Resources

According to the Healthcare Intelligence Network, seven states were recently awarded grants totaling $8.3 million to create information systems about long term care options for discharged hospital patients. For example, Oregon’s Lane County hospital will add a community case manager to give discharged Medicaid patients information about local community long-term services. It is a pilot program for services planned to be offered across the state.
These informational efforts will benefit the aging population and the family members who care for them. It will also require the addition of long term care experts at hospitals across the nation.
